Preparing for your office visit

When Booking an appointment, we may ask:
  • Who’s the appointment for? Please indicate if multiple patients
  • Your Date of Birth
  • Your Healthcare Information
  • Verify your personal information
  • Which team member you would like to see
  • The purpose of your visit

Appointments

Our staff attempt to contact all patients by either phone or automated text message reminders. If in the event you do not receive a phone call or text, please contact our reception team to confirm.

Things to bring to the appointment:
  • Health card, personal identification
  • Any health records, test results, or X-rays that you’ve been asked to bring
  • The name and phone number of who to contact in an emergency
  • A list of all medications you’re currently taking, including prescription drugs and non-prescription medications, as well as any vitamins, supplements, herbal and homeopathic remedies
  • A list of other health professionals you’ve recently consulted, why you went to see them, and what they told you.
Before you arrive, you should consider what you may want to ask your doctor:
  • What caused this condition?
  • What are my treatment options?
  • Which option is best for me, and why?
  • How will my lifestyle be affected, what will I need to change?
  • What am I doing right?

Update your information with Alberta Health:

Please visit the Alberta Health website (image below) to keep your health account information current if you move, have children, turn 21 or 65, get married, divorced or other relationship changes.

We accept cash, credit/debit, and e-transfer. Please send e-transfer payments to payments@taberclinic.ca
